Enrollment

If you are interested in enrolling your child, please fill out the form below or download a printable application.  A staff member will contact you as soon as possible with more information. 

You may also stop into the center nearest you at any time.  You will need to bring the following information in with you: proof of income, your child's birth certificate, shot record, and medical insurance. Physical and dental exam forms are available here to be printed or these forms can be given to you at any center.  These forms must be completed before the child can attend our program.

Holy Cross Head Start, Inc. is proud to collaborate with the Buffalo Public School District to offer its Universal Pre-Kindergarten (UPK) Program.  UPK will provide your child with a literacy-rich curriculum aligned with the New York State Standards and offers high quality learning experiences to prepare your child for Kindergarten.  To apply, call or visit one of our Buffalo locations.  If more requests for enrollment are made than can be placed, a random lottery will be used to choose children for enrollment in the UPK Program.  All other children will be referred back to the district or to other Community Based Organizations for enrollment.  In the event that all district slots are full, any remaining children will be placed on a waiting list.  Registration/application to the UPK Program is ongoing.  Once the Program is filled, all applicants will be referred to other programs or added to the program waitlist thoughout the remainder of the school year.
